DISCLOSURE OF COMPENSATION

In addition to the fees and/or commissions received by Florida Insurance Center, Inc. from
clients for placement of insurance coverage, in certain circumstances various companies
of Florida Insurance Center, Inc. may earn and retain usual and customary compensation via
fees and/or commissions for its role in placing insurance under separate agreements
with insurers and re-insurers.

Additionally, Florida Insurance Center, Inc. may receive contingent fees for placing coverage
with particular insurance companies from some insurers based on factors that are not
client specific, such as aggregate loss experience, or size or performance of an overall
book of business placed with an insurer.

contingency fees received under them is available upon request.
